Background

Liam is a 17-year-old with learning disabilities and mental health challenges who currently resides in ring-fenced shared accommodation as part of a supported living service. As he approaches his 18th birthday, Liam faces significant changes as he transitions to adult services. His needs include assistance with daily living activities, managing his mental health, and preparing for greater independence.

Intervention

The Flame Lily Healthcare developed a detailed, person-centred transition plan for Liam. This plan was crafted in collaboration with a multidisciplinary team, including social workers, occupational therapists, GPs, and mental health professionals.

To ensure Liam’s smooth transition, the plan incorporated:

● Assistive Technology (TEC): Tools such as a calendar app and visual planners were introduced to help Liam manage his schedule, including attending college, therapy sessions, and social activities.

● Supported Living Services: Liam’s accommodation was tailored to suit his needs, including a private bedroom in shared accommodation with peers who share similar health challenges. Communal areas fostered social interaction while staff provided guidance with daily tasks like cooking, cleaning, and budgeting.

● Skill Development: Dedicated staff worked with Liam to enhance life skills such as meal preparation, self-care, and navigating public transport.

● Social Integration: Staff supported Liam in finding part-time work and engaging in community activities to build his confidence and independence.

● Close Collaboration: Regular meetings with Liam’s social care and healthcare providers ensured continuity of care as he transitioned to adult services under the Care Act 2014.

Impact

Six months before his 18th birthday, Liam has shown tremendous progress. TEC tools have helped him gain confidence in managing his day-to-day responsibilities. With staff support, he has learned to prepare meals, budget his finances, and independently travel to college.

Liam is now actively participating in a community sports club, where he has made new friends and built self-esteem. The transition plan, grounded in collaboration with multiple agencies, ensures that he will seamlessly move to adult services with minimal disruption, preserving his progress and fostering long-term independence.
